Odontomas are the most common odontogenic tumors. Intraosseous odontome rarely erupt and very few cases have been reported in the literature. Here we are reporting a case of large erupted complex odontoma in the posterior maxilla, associated with an impacted second molar that caused secondary symptoms like pain and swelling. INTRODUCTION : The term odontoma literally means any tumor of odontogenic origin. However, odontomas are actually mixed odontogenic tumors because they are composed of both epithelial and ectomesenchymal components.1 According to the the second edition of the WHO Histologic Typing of Odontogenic Tumors, odontomas are classified under the broad category of tumors containing odontogenic epithelium with odontogenic ectomesenchyme, with or without dental hard tissue formation.2 Odontomas can be further subdivided into compound composite and complex composite types.The compound composite odontoma is defined as a malformation in which all of the dental tissues are represented in a more orderly pattern than in the complex composite odontoma such that the lesion consists of many tooth-like structures.3 The WHO defines complex odontoma as a malformation in which all of the dental tissues are represented, and individual tissues mainly are well formed but occur in a disorderly pattern .4 Clinically, compound and complex odontoma can be further subdivided into three types of odontomas: central odontoma, peripheral odontoma and erupted odontoma. 5,6 Intraosseous (central) odontomas are the odontogenic tumors of greatest incidence, representing 51% of all odontogenic tumors. Compound lesions occur predominantly in the anterior maxilla and complex lesions occur in the mandibular molar regions. Peripheral or extraosseous odontomas are defined as tumors with the histological characteristics of intraosseous odontoma but occurring only in the soft tissue covering the tooth-bearing portion of the mandible and maxilla. Rarely, intraosseous odontomas located coronally to an erupting or impacted tooth or superficially in bone may erupt into the oral cavity and are known as erupted odontome.5 Erupted odontomas in the oral cavity are rare, and the first case was published in 1980 by Rumel et al.7,8 According to a data published in december 2014, there have only been 20 reported cases of erupting odontoma in the literature, of which 11 were complex odontomas. 9 Hence, we report a case of a erupted Complex Odontoma lesion that was located coronally to the crown of an impacted right maxillary second molar.
